Bergdorf Goodman Evening Gown 1965

From my mother's vintage clothing collection. The construction is amazing. My questions are:
1. I can't figure out the material for the skirt. The lining is silk but the fabric is heavy woven with thick/thin lines. I would assume it is silk too?
2. The tag for the customer, Mrs. Cobb, would this indicate the dress was custom made or was tailored for her?

I agree with Marsha that this was probably a rtw dress and Bergdorf's altered it; however, I'm surprised that they put their custom salon label in it.
Bergdorf's custom salon was different from I. Magnin's (only made custom garments and couture copies). Bergdorf's had muslins of designs and fabric swatches from couture houses. Clients could place an order and Bergdorf's would tweak the fit as needed.
They made Jackie Kennedy's inaugural gown and I have a dress that was made for an inaugural ball.
There is a chapter in Bergdorf's on the Plaza that focuses on the couture salon.
